As a community-engaged coalition, we are actively exchanging our knowledge, ideas and strategies, with shared risks and responsibilities to build community concern and consensus around the needs presented in our Community Health Needs Assessment (CHNA), Community Health Improvement Plan (CHIP) and Community Listening Sessions. Our members engage in collective actions that build strength, improve trust and elevate communication among our local community sectors. By mobilizing our diverse talents and resources we are able to build the capacity needed to implement culturally appropriate health improvement strategies that meet our community's specific needs, increasing community ownership and long term sustainability.
Allendale County HEALing Partners operates with a high level of trust, reciprocity, support and ownership among community members. Sectors are actively involved in the strategic planning and implementation of our identified health-focused strategies. Our meetings are productive with decisions being made by the members. Evaluation is a key part of an ongoing assessment in Results Based Accountability and we are supported by frequent training opportunities to increase our knowledge as an engaged group through the South Carolina Office of Rural Health (SCORH). It's a great place to learn and grow and membership is open to all.

our most current Community Health Needs Assessment (CHNA)
A Community Health Needs Assessment (CHNA) is a systematic process involving our own local community in identifying and analyzing our community's health needs. The process helps provide a way for local communities to prioritize their specific health needs, and to plan and act upon unmet community health needs. Our local hospital, as a designated Critical Access Hospital (CAH) conducts a CHNA once every three years.
