Allendale County Healing Partners, (ACHP), formerly named, Wholespire Allendale County, was formed in response to the 2019 Community Health Needs Assessment identifying our community as one of the greatest in need, ranking 45th of 46 counties as one of the least healthy counties in South Carolina. Our coalition follows the Collective Impact (CI) framework to improve health outcomes for the residents of Allendale County.
Organizations from different sectors of the community have come together with a shared vision to address these identified community health needs and have agreed to collaborate using the CI model to assure equitable opportunities to improve health and well-being for all residents. The coalition includes community-based organizations, (CBOs), health care providers, businesses, state and regional support agencies, institutions (schools, colleges, universities), faith-driven organizations, and individuals with lived experience.
ACHP’s scope and purpose are to improve health outcomes in line with the 2022 Community Health Needs Assessment (CNA) and the 2023 - 2025 Community Health Improvement Plan (CHIP). The mission is to provide our community with proven and sustainable approaches that lead to increased access to healthy choices for all people. The vision of ACHP is to have informed influencers and an empowered community working together to ensure an equitable Allendale County, where everyone has access to healthy choices.
Using the CI model, we will align our activities and leverage our resources toward the common goals of addressing our identified community health needs. ACHP has developed a common goal and mission statement which is read before each meeting. We have regular monthly meetings and subcommittee meetings (working groups), all with open discussion to reach consensus in collective impact. We have scheduled county wide listening sessions to ensure meaningful community engagement.
Our service area is Allendale County, SC which is neighbored by Barnwell and Hampton Counties. The county covers 408 square miles and is the least populous county in South Carolina. Based on data found at Census.gov from the 2020 Census, the population of 8,039 represents a 23% decline since the 2012 Census.
